Examples of these health care delivery systems are long term care, home care, in-patient care, out-patient care, mental health care.One of the most common and needed in this aging world is the long-term care delivery system. This care is made of variable services that focus on both medical and non-medical care especially formed for people with persistent diseases and disabilities (“Long-term care”, 2007).

Long-term care can be delivered to your home, the community, and in assisted or in nursing homes (“Long-term care”, 2007). It helps people to regain back their everyday life and routine activities like dressing and taking a bath. Thus, this care facilitates in providing services that helped patients in their personal needs. If this is the case, long-term health care is available and needed in any age of a person’s life. There are many types of long-term health care and this includes Community-based Services, Home Health Care, In-Law Apartments, Housing for Aging and Disabled Individuals, Board and Care Homes, Assisted Living, Continuing Care Retirement Communities, and Nursing Homes (“Types of long-term care”, 2007). Each type has its own payment processes and paper requirements to enjoy such care with its corresponding services.The community-based service is readily available from the community itself. This has different programs and services that helped elders and people with disabilities and chronic diseases in their different personal commotions.

Among these services are Meals-on-Wheels, transportation services, personal care, chore services, adult day care and a variety of activities in senior centers (“Types of long-term care”, 2007). These are usually available as specially discounted or free to qualified individuals. There is a local organization called Area Agencies on Aging that manages and facilitates these services to endorse and uphold the needed independence and dignity of these patients under long-term health care especially the elders. The State Medicaid, a health insurance provider, can assist in the payment for home-based care like nursing care, home health care, personal care, chore services, and medical equipments (“Types of long-term care”, 2007). But application of such insurance still depends on the qualification of the applicant. The community-based health care services available in a community yet still vary. These services may all be available in your community or just some of it. Examples of these services are Adult day care, Senior centers, Financial management, Transportation, Meals On Wheels, Telephone reassurance, and Case management (“Types of long-term care”, 2007).

Another type of long-term health care is the home health care. This is dependent of the assistance that the patient needed. This is usually decided with the help of the family members of the degree of need for a home health care. This care is commonly needed in case of certain illnesses and injuries wherein daily activities are impeded. The home health care is provided by a duly licensed health worker like a registered nurse, practical nurse, or a therapist. This care is usually associated of skilled nursing care. The payment assistance is also available in home health care through the Medicare.

But certain requirements must be satisfied to avail it. In case where home health care is not covered under Medicare, there are available home health care agencies that can provide same health care service. But for this, you have to pay it for your own. The cost for home health care depends on the location of the patient, type of home health care service needed, and the frequency of the service to be done.

Usually, this type of care is charged per hour. Like in community-based services, home health care services available vary in different areas. Examples of these services are Skilled nursing care, Homemaker/Health aides, Personal care aides, Respite care, Medical equipment, Home repair and modification, and Hospice.Another type of long-term health care is In-Law Apartments. This mandates a separate housing that may be within a home or on a lot. It is referred by other references as accessory apartment or dwelling unit. Usually, after the use of the in-law apartment, this can be used as a dwelling unit for the caretaker or for rental to provide an additional income. But before making this as an additional income source, ask your respective local or state government because some state has restrictions on building an accessory apartment in the property other than care service purposes.The Housing for the Aging and Disabled Individuals is a federal and state government program that helps the elders under low to moderate incomes ($46,000-$53,000) in the payment of their housing. This housing program also includes assistance in meals and other routine activities that cannot be done by seniors already like laundry. The qualified senior usually live in an apartment in a complex.

The monthly income and expenses are further studied and investigated if the senior is qualified for the housing program. The payment is a percentage of the senior’s income.The board and care home is a living arrangement to provide care for people incapable of doing their daily activities. These patients do not need nursing care services but just assistance in their routine activities. The payment is done through long-term insurance and other assistance programs and charged as percentage of the income. This is also called group home. The assisted living is also similar to board and care home but recreational activities are also provided.

The patients often live in their own room or own apartment that is located within a building or in a group of buildings (“Finding a nursing home”, 2006). These patients share all or some meals together. The cost of this care depends on the living area size, services rendered, type of help asked, and the building location. The fee charged is the monthly rent and additional fees for other services.

This fee usually ranged from $900 to $3,000 per month. More expensive fees are collected in urban areas or in chic facilities.The continuing care retirement communities (CCRC) provide different degree of services depending on the level of need and care that the retiring individuals. Typically this ranged from assistance from daily activities to nursing care.

As the level of care increases and become more critical, the residents also enjoy this benefit but still stay in the CCRC. The entrance fees ranged from $38,000 to $400,000 and monthly payments from $650 to $3,500. The very extensive range of services includes personal care, health services, and support services. This is not covered under Medicare.

The availability of long-term care minimizes the least care especially on health care for our senior citizens. The pity and abandonment that are not supposedly to be happening for our elders because they are like disposed things not needed anymore as they are old will be prevented.
